PHYSICIAN AND HOSPITAL REFERRAL PROGRAM
With access to a network of thousands of health care provider nationwide, members can save on medical services. Members who have no health benefits can save money on all visits to a participating provider. Members who do have health care can save on services not covered, such as routine office visits and cosmetic surgery. At a savings 5%–40%, members can save hundreds or even thousands of dollars a year.
Participating providers are screened by a team of professionals and must pass strict criteria prior to joining the network. All providers are licensed and credentialed. There is no paperwork to compile and no pre-certification requirement. There is no limit to the number of visits to your provider. Simply make an appointment with a participating provider from the list in your membership kit or contact a member services representative at 1-800-308-0374 for the names of additional providers in your area. You may also use the Internet to search for participating providers online.
For physician visits, present your identification card and the “Dear Doctor” letter included in your membership kit to the receptionist when you arrive at the participating physician’s office. For services performed by the physician, you will pay the reduced fee at the time of your visit.
For services performed in the hospital, (only if “Hospital” is on your identification card), you will receive a billing statement detailing your savings and the amount you must pay within 30 days of services.
Remember, this is not an insurance plan and there will be no reimbursement to you or to the participating provider. You are responsible for paying the provider directly for services rendered.
This program is provided through one of the largest provider networks in the nation. The network is available throughout the United States and providers are continually being added to meet your member needs.
