Benefits

Chiropractic
The Chiropractic Plan provides nationwide access through a network of licensed and insured chiropractors. The benefit includes a free initial consultation, flat fee discounts on examinations and x-rays, forty percent (40%) savings on diagnostic services, twenty percent (20%) savings on all other services and follow up treatments, and no limit on the number of visits.
Dental
The Dental Plan provides nationwide access through a network of credentialed practitioners including general dentists and specialists. Members can save on routine and preventive care in addition to more extensive treatments such as fillings, crowns, root canals, orthodontics, and cosmetic dentistry. Although discounts may vary based on geographic region, the average range of discount is between 10% - 50%.
Hearing
The Hearing Plan provides nationwide access through a network of licensed audiologists. Members receive a 10% - 20% discount from provider’s ordinary and customary fees for hearing aids. Free annual cleaning and check of hearing aids purchased through the program is also included.
Prescription Drugs
The Prescription Drug Plan is offered through a nationwide network of pharmacies, including major chains. The benefit provides an average of 20% discount on most brand name and generic prescription drugs; in addition, a mail order prescription drug service is available, offering discounts between 15% - 50% on most prescription drugs.
Vision
The Vision Plan provides nationwide access for purchases of corrective eyewear, contact lenses and related sundries. Network providers are appropriately licensed, certified or qualified under state law. Network locations include several nationwide retail outlets. Members receive a 20% - 60% discount on eyewear products and substantial discounts on mail order replacement contact lenses. In addition, Members save 10% - 30% on eye examinations and surgical procedures, including Lasik where available, at participating providers.
Vitamins and Nutritional Supplements Plan
The Vitamin and Nutritional Supplement Plan is offered online or by mail order nationwide. Members can receive savings of up to 30% off catalog prices and an additional 15% on already reduced prices when shopping online. Products include non-prescription medications, beauty supplies, nutritional supplements and more.
24-hr Nurse Helpline Plan
Through the 24-hr Nurse Helpline Plan, Members have access to Registered Nurses via a toll-free phone number 24 hours a day, 365 days a year. These nurses are specially trained to offer prompt, confidential medical counseling.
Physician and Facilities Plan
The Physician and Facilities Plan provides access to thousands of qualified, credentialed medical facilities and physicians nationwide with reductions of 5% - 40% off all services. This benefit is not available in Maryland.

The plan member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with the discount medical plan organization.
